Instead, how about voters being required to pass a cognitive test before voting?

They tried that; it was called "Jim Crow".
Not as a blanket rule, no. It would become highly politicized, for sure. If there are questions about a candidate's cognitive state, the candidate can decide for themselves if they want to get tested and share the results. If not, they'll feel those doubts at the ballot box.
